SURGERY CLOSURE

Patient Survey 2009/2010

The patient survey will be carried out by the Belfast Health and Social CAre Board and some patients will receive the form through the post in January/ February 2010. Thank you for taking the time to complete this form. The feedback will enable us to inprove our service to our patients.

Ordering Repeat Prescriptions

We have a dedicated phone number for repeat prescription requests.  The number will be 028 90813266 and it is available almost 24 hours per day, 7 days per week.

When you ring you will be asked for your name, your date of birth, your address and what medications you want to order.

The requests will be processed each working day between 11am and 1pm (during this time the service will not be available) and your prescription will be ready for collection within 48 hours (2 working days).

We are not able to process repeat prescriptions on the normal surgery telpehone number.

The practice is located in purpose built premises on the corner of the Ballynahinch Road and Hillsborough Road. The car park, which is shared with the pharmacy, is accessed from the Hillsborough Road. The practice area is approximately within a 5 mile radius of the surgery. The practice premises have access and facilities for all disabled persons.
