Before registering with the practice you must complete a couple of forms.  You can download the Carryduff Surgery Forms below.  Please download these, print them out and fill them in before visiting the practice for the first time. Thank you

When you visit the practice for the first time you will be asked to complete a CSA (Central Services Agency) registration form if required.

The practice is located in purpose built premises on the corner of the Ballynahinch Road and Hillsborough Road. The car park, which is shared with the pharmacy, is accessed from the Hillsborough Road. The practice area is approximately within a 5 mile radius of the surgery. The practice premises have access and facilities for all disabled persons.
