Changes to Travel Vaccination Procedure  

Patients planning to travel abroad that require advice about vaccinations should complete a Pre-Travel Risk Assessment form and submit it to the nurse along with your telephone contact details. The nurse will then assess what vaccinations are necessary and contact the patient regarding an appointment.  If you have not heard from the nurse within two weeks, please contact the surgery as we may have been having difficulty getting in contact. Forms can be collected at the front desk or downloaded from our travel information section of the website.
